Transformation Series Program Policy & Treatment Compliance Agreement

Program Commitment Policy

Our Transformation Series programs are medically guided treatment plans designed to deliver optimal results when followed as prescribed by your licensed medical provider.

By enrolling in a Transformation Series or bundled treatment package, you acknowledge and agree to the following terms:

Strict No Refund Policy

All Transformation Series programs, bundled packages, memberships, and prepaid treatment plans are NON-REFUNDABLE.

This includes, but is not limited to:

• Body contouring series

• Fat reduction protocols

• Hormone optimization programs

• Peptide programs

• Skin rejuvenation series

• Multi-session aesthetic treatment plans

Once payment is processed and services are reserved, no refunds will be issued for any reason, including but not limited to:

• Change of mind

• Scheduling conflicts

• Personal circumstances

• Dissatisfaction due to incomplete protocol adherence

• Failure to complete recommended sessions

Medical Treatment Plan Compliance

Your treatment plan is customized by a licensed medical provider based on your health history, anatomy, and clinical assessment.

For optimal outcomes, you agree to:

• Follow the recommended number of sessions

• Attend treatments at prescribed intervals

• Adhere to pre-treatment and post-treatment instructions

• Maintain recommended lifestyle, hydration, nutrition, and aftercare guidelines

• Complete the full protocol as advised

Deviation from the provider-recommended plan may result in:

• Reduced or delayed results

• Suboptimal aesthetic outcomes

• Need for additional treatments at additional cost

Results Disclaimer

Individual results vary and cannot be guaranteed.

Outcomes depend on:

• Biological response

• Compliance with medical recommendations

• Lifestyle factors

• Consistency of treatments

Failure to follow the prescribed plan voids any expectations of outcome performance or corrective services.

Missed Appointments & Treatment Gaps

Missed appointments, late cancellations, or extended gaps between sessions may negatively impact results and are not grounds for refunds or credits. A no show fee of up to 50 % of book service/ or treatment transformation will apply .

Transferability Policy

Transformation Series packages are non-transferable and may not be shared or reassigned to another individual. They must be redeemed within 6 months of purchasing .

By signing below, you confirm that you:

• Understand the medical nature of the Transformation Series

• Agree to the strict no refund policy

• Accept responsibility for following provider recommendations

• Understand that results depend on compliance and biological response

• Consent to proceed with treatment under these terms
